When a full time staff uses sick leave to care for family
members due to his/her bona fide illness, disease, or injury and for visits
with health care providers, the employee should complete this form http://www.houstontx.gov/hr/email/images/Sick%20Leave%20Form%20PDF%20Fillable%20Saveable%209-13-2013.pdf
to affirm that the family member qualifies as a family member for sick leave
purposes. The forms should be submitted to supervisors and a copy filed
in the HR Office. Family members include spouse, child or step-child, and
parent or step-parent.
