Greetings - Below is reminder information on how all incident reports should be named when submitted.
This information will be shared in your managers’ meetings and added to the NL blog. To facilitate filing and finding incident reports throughout the system, HPL is adopting a naming convention for these reports.
Please name all incident reports in the following manner: Date-location-staff initials-subject-name of person involved. For example, if, yesterday, staff member John Doe at Robinson-Westchase filled out an incident report about customer Martin Smith assaulting the security guard it would be named: 051610-ROB-JD-Assault-Smith.doc If you don’t have a person’s name, just leave it off.
All reports will be able to be viewed on the G:\ drive in the G:\INCIDENTS files. Please continue to put your location code and a brief summary of the incident on the subject line of the e-mail.
